Directions
From 57th and Western go South 6 blocks.  Southwestern Dental is a landmark on your right hand side.  Turn right and come West 1/2 Block to the 2121 Building which sits just behind the Southwestern Dental building.  Come in the East door, which enters the lower level of the building.  Create Therapeutic Solutions, LLC is in Suite 40.
